Insurance, payment
Mainly a super bill will be
provided for you at your
request to assist you in
filing. Some insurance
claims may be filed on
your behalf after proof of
your deductible has been
met. I accept all PIP
(personal injury
protection) and some
major medical; Mainly
BCBS of Florida, City and
State plans. For major
medical it's best that you
call your carrier by dialing
your member services
number found on the back
of your card, and verify
your benefits for
acupuncture and
coverage prior to
treatment. Help may be
provided if you need
assistance.
Acupuncture

are recognized as
Primary Care Providers.
No prescription or
referral is required for
Acupuncture services.
Companies that offer
discount programs. You
will not see my name on
their list. However, if your
company offers
discounted services for
alternative care, I'll do my
best to honor their
discount. Just call prior to
coming in. I'll either match
the discount rate or help
you to choose the best
practitioner from the list of
ones that do.
For those without
insurance coverage, my
rates are competitive to
the area. Rates are
according to the duration
and modality. A sliding
cash scale is provided to
medicare / medicaid
recipients as I do not want
cost to interfere with you
receiving the treatment
that you need.
